9. To maintain peace and order in my community.

Procedure in Filing SSS UMID (Unified Multi-Purpose ID) Application

*NOTE: No Service Fees In Processing UMID Application.

1. Get UMID (Unified Multi-purpose ID) Application Form from the Member Service Representative at the General Information Center (MSS) usually located at the Ground Floor of SSS Branch, or download it from SSS Website (www.sss.gov.ph).

2. Read instructions and fill out the form.

3. Get a Transaction Slip from the Member Service Representative at General Information Center (MSS) located at the Ground Floor and wait for your turn at the assigned counter.

4. Submit the properly filled out UMID Application Form together with one (1) of the primary documents or in its absence, two (2) valid ID's with one bearing signature and photo to the Member Service Representative at the MSS of the SSS Branch.

Primary documents are: Driver's License, Passport, Professional Regulation Commission (PRC) card, Seaman's Book.

5. Get the acknowledgement stub from the Member Service Representative at the MSS of the SSS Branch or from SSS Website (www.sss.gov.ph).

A. Hanapin at linisin ang maaaring pamugaran ng mga lamok at gawin ang mga sumusunod:

- Butasan, biyakin o kaya ay lagyan ng lupa ang mga lumang gulong upang hindi maipunan ng tubig at pangitlugan ng lamok.
- Takpan ang mga drums, timba at iba pang ipunan ng tubig upang hindi pamahayan ng kiti-kiti at linisin ang mga ito minsan isang linggo.
- Palitan ng tubig ang plorera o flower vase minsan isang linggo.
-Linisin at alisin ang tubig sa paminggalan.
- Itapon ang iba pang bagay na maaaring pag-ipunan ng tubig na pangitlugan ng lamok tulad ng lata, bote at tansan.
- Linisin ang alulod ng bahay upang hindi maipunan ng tubig at pamahayan ng kiti-kiti.

B. Magpatingin sa Health Center kung may sinat na ng dalawang araw.
